Minot, NDRENEWAL NOTICE FOR DENTISTS, HYGIENISTS, AND ASSISTANTS
North Dakota statutes governing the practice of dentists, hygienists, and assistants stipulate that to maintain your North Dakota license/registration, you must submit continuing education and renew your license/registration no later than December 31, 2009. Your license/registration will expire December 31, 2009, if not renewed.
ONLINE RENEWAL: You may renew online by clicking here. (Please link to Online Renewal) You will need to have your license number, Social Security Number, a Visa or MasterCard debit card or credit card and have the required hours of continuing education already recorded. You must enter your first and last name as it appeared on your renewal notice. If the required continuing education is not recorded and Infection Control and CPR were not updated within the last 2 years, you will need to mail or fax the completed CE forms to the Board office before renewing online. You may also check your continuing education by clicking "Continuing Education Status" on the bottom left. If you do not wish to renew your license online, you can complete the enclosed renewal application form and return it with the appropriate renewal fee postmarked no later than December 31, 2009.
Renewal Applications received online or postmarked after December 31, 2009 will be charged the additional late fee. There are no exceptions to the late fee. If you do not wish to renew, please indicate such on the renewal form and return it to the address listed your renewal notice. Your license expires December 31, 2009. After that date, you will not be able to practice until your renewal form, fees and continuing education have been received. If the completed renewal has not been received by February 25, 2010, your license will be cancelled.

IMPORTANT INFORMATION BELOW FOR DENTISTS AND HYGIENISTS
DENTISTS - ANESTHESIA ENDORSEMENT: If you are renewing an anesthesia endorsement for general anesthesia or deep sedation, moderate sedation (parenteral or enteral), or minimal sedation, you may renew online or submit an application for renewal by requesting an application from the Board or print the Renewal Application. The renewal fee for anesthesia endorsement is $200. Initial applications for anesthesia and sedation are available for download or by request. The ADA recently modified it's guidelines for the use of sedation by appropriately trained dentists. Please review the ADA's Guidelines For The Use of Sedation and General Anesthesia For Dentists. The NDSBDE has amended its requirements and application process for anesthesia endorsements accordingly. Information is available on the Board's website or by contacting the ADA directly for the Guidelines for Sedation. If you do not have an anesthesia permit, but wish to apply, please complete the Initial Application for anesthesia endorsement.
HYGIENISTS - ANESTHESIA ENDORSEMENT: Before administering local anesthesia, a dental hygienist must successfully complete a didactic and clinical program sponsored by a dental or dental hygiene school accredited by the Commission on Dental Accreditation, resulting in a dental hygienist becoming clinical competent in the administration of local anesthesia. A dental hygienist must apply for a permit to administer local anesthesia by completing an Application for Local Anesthesia Permit.
DENTISTS AND HYGIENISTS - INACTIVE STATUS: If you practice dentistry out of state or have retired, you may wish to consider the inactive status to retain the license in good standing and avoid cancellation. Upon payment of the inactive status license fee of $35.00 annually, a licensee may request to have their license placed on 'inactive status' upon expiration of the license. While on inactive status, the licensee may not engage in the practice of dentistry in the state until the individual meets the reinstatement requirements set by the Board. Please review the Reinstatement Requirements and print the Application for Inactive Status or contact the Board office to receive an application.
DENTISTS AND HYGIENISTS - RANDOM CONTINUING EDUCATION AUDIT: You may be selected for a random audit of your continuing education. Please keep materials for the previous and current CE cycle that are pertinent to your CE coursework as "proof" in the event of an audit. Examples of items to save are receipts, registration materials, certificates, or cancelled checks.

Name and Address Changes:
The Board requires that you submit your name and address changes in writing within 30 days of the effective date of the name or address change. For a name change, the Board requires a notarized copy of the official paperwork (such as a marriage license or other appropriate court document) showing the name change. Name changes cannot be done over the phone, or by email or fax! Please also include your former name and license number. Please complete the name and address change form.Purchase Licensee List:
The Board requires a written request or completion of the request form and $100 fee. There is a separate fee for each profession. The fee is $300 for all three professions. You may mail your request and fee payable to NDSBDE, PO Box 7246, Bismarck, ND 58507. In your written request, please indicate the purpose of the list (continuing education, research, recruitment), the order (alpha, ZIP, by city, country, or specify order) and format (email, labels, or list) that you wish to have. For emails, please indicate the file format (Excel or delimited). We cannot provide sensitive information such as SSN, date of birth, or home phone number. We will provide date license, license number, expiration date, etc.
