ATTENTION DENTIST and HYGIENIST APPLICANTS: Please read important information below before applying for licensure!
All license applications for dentists and hygienists must be received in the Board office 30 days prior to the next Board meeting (North Dakota Century Code, Chapter 43-28-11).
The next formal meeting of the North Dakota State Board of Dental Examiners is:
Next Meeting of the Board
Thursday, Sept 17, 2009
8:00 a.m.
Holiday Inn
3803 13th Ave So.
Fargo, ND
Effective January 1, 2008, according to NDCC 43-28-11.2, all new and reinstating applicants for licensure as a dentist or hygienist shall be required to submit to a statewide and nationwide criminal history check. A Criminal Records Check (BCI&I and FBI) completed by the Bureau of Criminal Identification and Investigation, must be submitted directly the Board. Once the Board office receives your application for license, a Criminal History Record Check Request form and two (2) fingerprint cards will be mailed to you. Complete the form and mail it back to the Board office along with the two (2) fingerprint cards containing your set of prints. Both cards must be printed on and submitted directly to the Board office. All costs associated with obtaining a background check are the responsibility of the applicant. The cost to run the criminal history checks is $47.25. You must submit a check or money order for $47.25, made payable to the North Dakota Attorney General along with the form and both fingerprint cards to the Board’s office. Local law enforcement professionals must take fingerprints. Please call ahead, as fingerprinting may only be offered on certain weekdays. Results shall be received by the board prior to the issuance of a license to practice. Turn around time for this process takes at least two weeks. Keep this in mind during the application process.
If there are items on the application about which the Board desires additional information, or if there is a malpractice history, or a discovery on the criminal background check process, the application will take longer to process.
Your application will be kept on file for one year from date of receipt. Your file will be purged if you do not complete your application process in one year. You will need to resubmit a new application packet and fee after that time.
We recommend that applicants handle their own applications without delegating this task to someone else. In our experience, delegation frequently does not work well. The process seems to work more smoothly when individual applicants personally take responsibility for their own licensure and application process.
License Expiration Grace Period for New Applicants: All new applicants who are issued a license within 120 days of the upcoming renewal expiration date will be issued a license expiration date of the next renewal period.
