Dentists | Hygienists | Assistants

Dentist:

  • Must submit application to the executive director on:
    NDSBDE Application for Dental License  /  Application Checklist
  • Must be a graduate of an ADA-accredited dental college.
  • Must submit application fee of $440.00/new graduate or $1200.00/credential review.
    License Fees are nonrefundable
  • Must successfully complete the ND Jurisprudence exam.
  • 2009 Clinical Examinations: The NDSBDE accepts the revised 2009 WREB exam in addition to the CRDTS exam for ND licensure. ADEX dental exams taken after June 30, 2009, will no longer be accepted by the NDSBDE.


Hygienist:   

  • Must submit application to the executive director on:
    NDSBDE Application for Hygiene License  /  Application Checklist
  • Must be a graduate of an ADA-accredited dental hygiene school.
  • Must submit application fee of $200.00/new graduate or $450.00/credential review.
    License Fees are nonrefundable
  • Must successfully complete a board-approved national examination. (CRDTS, WREB, and SRTA exams are accepted).   ADLEX (ADEX dental hygiene) exams taken after December 31, 2008 are not accepted.
  • Must successfully complete the ND Jurisprudence exam.

Assistants:

Registered:

  • Must submit application to the executive director on:
    NDSBDE Application for Registered/Qualified Dental Assistant.
  • Must submit $65.00 application fee. License Fees are nonrefundable
  • Must have CPR and Infection Control in the last 24 months. Submit verification of CPR. Verification of completion of Infection Control course if not a graduate of a dental assisting school in the last 2 years or took the DANB exam more than 2 years ago.
  • Must Submit One or More of the Following:
    • Verification of certification by the Dental Assisting National Board (DANB)
    • Verification of completion of ADA-accredited program in Dental Assisting
    • Verification of completion of a course in Dental Assisting approved by NDSBDE
  • Verification from other jurisdictions in which registered, if applicable
  • Must complete the Jurisprudence Exam. Please contact the Board office to have an exam and Rules book sent to you.

Qualified:

  • Must submit application to the executive director on:
    NDSBDE Application for Qualified Dental Assistant.
  • Must submit $65.00 application fee. License Fees are nonrefundable
  • Must submit verification of being employed and trained as a dental assistant for at least 6months working at least 24 hours a week.
  • Must have CPR and Infection Control in the last 24 months. Submit verification of CPR.
  • Must submit verification of successfully completion of a board approved infection control seminar and passed the x-ray and infection control portions of the DANB exam.
  • Must complete the Jurisprudence Exam. Please contact the Board office to have an exam and Rules book sent to you
